The 5 Best Intake Form Software for Wellness Coaches, Compared

The 5 Best Intake Form Software for Wellness Coaches, Compared

The best intake form software for wellness coaches does more than collect a name and a goal. It screens for the health issues that sit outside coaching, captures sleep, stress, movement and eating patterns in one pass, gets an agreement signed before the first call, and then puts those answers in front of you every week while the client works through a program. If a tool only stores a PDF of answers that nobody reopens, it is a form builder, not intake software for the way wellness coaching actually runs.

Our pick is Coachful, because the intake questions, the agreement, the payment, the program and the daily check-ins live under one login and one client record. We make Coachful, so read the criteria below and judge the ranking on them. Four other tools are worth knowing: Dubsado for form-heavy admin, Quenza for worksheets between sessions, Practice Better for practitioner-style charting, and HoneyBook for proposals to employers buying wellness workshops.

Every price below for the other tools is as listed on their own pricing pages in October 2026. We have not run client programs inside these tools; we describe what each vendor documents and what public reviewers report.

What do wellness coaches need from intake form software?

Wellness coaches need intake software that handles a whole-person picture, flags anything outside coaching scope, and keeps answers attached to the client for the length of the program. That is a different job from a life coach's values questionnaire or a personal trainer's readiness screen, because a wellness client often brings sleep, stress, energy, digestion, movement and emotional load to the same first call.

Look at how most wellness offers are built. A typical practice runs a free discovery call, then a paid 8 to 12 week reset or habit program, sometimes a small group cohort, and often a corporate workshop or a retreat on the side. The intake has to serve all of those, and it has to do four specific jobs.

Screen for scope before the first session

Wellness coaching is not medical care, nutrition therapy or psychotherapy. Health coaching, as the overview of the field on Wikipedia describes it, works on behavior change alongside a client's medical care, not in place of it. Your intake needs questions that surface red flags early so you can ask for clearance from a doctor or refer out. The common ones:

  • Chest pain, fainting or dizziness with exertion, or a diagnosed heart condition (screens like the PAR-Q+ cover this; paraphrase its themes, do not copy it).
  • Pregnancy, recent surgery or an injury still under treatment.
  • A current or past eating disorder, or rapid unexplained weight change.
  • Recent medication changes, especially for blood sugar, blood pressure or mood.
  • Thoughts of self-harm, or symptoms of depression or anxiety that need a licensed clinician.

Write a plain scope line on the form itself: "I am a wellness coach, not a medical provider. I do not diagnose, treat or prescribe. If your answers suggest a medical or mental-health need, I will ask you to see a qualified professional before or alongside our work." Our wellness intake form template has a full question set you can copy.

Thinking of launching your coaching business?

Answer a few questions and Coachful builds it around you: your website, programs, booking and payments. Ready in minutes.

Build your platform

Capture lifestyle in a way you can reuse

A wellness intake asks about bedtime and wake time, caffeine and alcohol, steps or movement minutes, stress triggers, energy dips, meals and hydration. Those answers are only useful if you can see them in week six when the client says "nothing has changed." Software that stores answers on the client record beats software that emails you a PDF.

Get the agreement and payment done before the call

Clients who sign and pay before session one show up. The intake software should send the coaching agreement for e-signature, take the payment, and only then release the booking. For the contract side, see our guide to coaching intake form and agreement software.

Keep health data where it belongs

Most wellness coaches are not covered entities under HIPAA, which applies to health plans, clearinghouses and providers who bill electronically (the HHS covered-entities guidance explains who is in scope). If you are also a licensed clinician, or you work inside a clinic, clinical records belong in a clinical system. If you coach people in the EU or UK, health answers count as special category data under the GDPR, so collect only what you need and say why. Rules vary by country and state; check with an attorney if you are unsure where you fall.

Which tool wins for each wellness coaching situation?

Coachful wins overall; the other four each win one narrower situation. Ranking criteria: one login for the whole client journey, delivery between sessions, total stack cost per month, and setup effort (including Coachful's free white-glove build).

ToolBest forStarting priceWhy it wins that scenario
CoachfulBest overall: intake that flows into a program, habits and check-insLite: $29/month for up to 5 clientsIntake answers, agreement, payment, program and daily check-ins on one client record
DubsadoCoaches who want form-driven admin workflows$35/mo StarterForms, contracts, invoices and automated workflows in one CRM
QuenzaWorksheets and reflective activities between sessions$25/mo Spark (10 clients)Activity and worksheet library with a client app
Practice BetterCoaches who chart like practitioners or trial with 3 clients$0 Sprout (3 clients)Charting, telehealth and a client portal built for wellness practitioners
HoneyBookSelling workshops to employers with proposals$29/mo Starter (billed annually)Proposals, contracts and invoices in one client flow

How did we choose these five tools?

We started from the wellness client journey (discovery, intake, agreement, payment, onboarding, weekly work, re-assessment) and asked how much of it each tool covers without a second subscription. Then we weighed four things:

  1. One login for the whole journey. Every extra tool is another place an intake answer can get lost.
  2. Delivery between sessions. Wellness change happens in the 167 hours between calls, so we favored tools where intake goals turn into habits, check-ins or worksheets.
  3. Total stack cost. A cheap form builder that forces you to add booking, payments and a program tool is not cheap.
  4. Setup effort. Wellness coaches tend to be solo. A tool that takes three weekends to configure costs real income.

We excluded pure form builders, because they stop at collection, and we left out general practice suites built mainly for insurance billing. Our wider roundup of software for wellness coaches covers the full stack beyond intake.

1. Coachful: best overall for wellness coaches

Scenario it wins: you run paid programs, not just one-off sessions, and you want each client's intake answers to drive what they do every day for the next 12 weeks.

A tablet showing a client's coaching hub with the next session and a pre-session item, beside a yoga mat and journal

Coachful is a coaching platform where intake is the first step of the client record rather than a separate form. One membership covers booking, payments, coaching agreements, programs with habits and check-ins, courses, a community, a website with funnels and email, and Michelle, the built-in AI assistant. The coach has one login; the client gets one app under the coach's branding inside the Coachful app.

How intake works inside Coachful

There are three places to collect intake answers, and wellness coaches usually use all three:

  • Intake questions on booking. Add questions to a session type, so a discovery call cannot be booked without the basics: main goal, sleep pattern, stress level, and the red-flag screening questions.
  • Application funnels. For a higher-ticket reset program or a cohort, an application funnel collects a longer form and puts the person into your leads list with the form as the source.
  • Forms and worksheets. Forms & Worksheets is one of the resource types, so a full lifestyle questionnaire can be assigned after the agreement is signed and before session one.

The Intakes page then tracks every intake call, with totals for upcoming calls, converted clients and conversion rate.

Here is a short walkthrough of the intake flow:

From signed agreement to first habit

Coaching agreements go out for e-signature before the first call and are stored on the client record. Payments run through your own Stripe Connect or Razorpay account, with one-time payments, subscriptions and payment plans. Coachful charges a 0% platform fee; only Stripe or Razorpay processing applies.

Once the client is in, the intake answers sit on the client record next to goals, check-ins, habits, notes, tags, payments and sessions. Michelle can brief you before each session from that live data, so the client who wrote "I sleep five hours and wake at 3 a.m." on intake gets a question about sleep in week four without you digging for it. Each client also shows a status of thriving, active, quiet or at risk, which is useful when a wellness client goes silent after a stressful week.

The client sees their next session and any required pre-session item in their coaching hub, then works through daily tasks, habits and morning and evening check-ins.

For coaches who also do movement or food work, the Fitness & Nutrition pack adds a workout builder, nutrition targets, meal plans, progress photos and weekly check-ins. You attach your own demo videos; there are no stock exercise videos, and there is no Garmin, Fitbit, Whoop or Oura sync.

What Coachful does not do

Coachful is not HIPAA-certified and does not sign a BAA, so it is not the place for clinical records or insurance billing. It has no PayPal, no permanent free plan, and no routing forms that pick a host from form answers. If you are a licensed dietitian or therapist who also coaches, keep clinical notes in your clinical system and use Coachful for the coaching side.

Cost and how to start

Plans are flat monthly, lower plans cap the number of clients, Pro is unlimited, and monthly plans cancel in one click (current plans: Lite: $29/month for up to 5 clients; Solo: $49/month for up to 20 clients; Pro: $99/month with unlimited clients; Studio: $199/month with unlimited clients). There is a free trial, and your existing clients, programs and content are imported for you during onboarding. Full client data, including form responses, exports as a ZIP.

Why Coachful stands out: a free white-glove service. From Create, the "We'll build it for you" option lets you describe what you want and attach up to 8 reference links and 10 files. The Coachful team builds it inside your own account, usually within about 24 hours. For a wellness coach that might be the intake form itself, a discovery-call booking funnel, a 12-week reset program or a calm, sage-and-cream website. One recent build: a menopause and midlife health coach starting from nothing got a five-page site in sage and cream with every button leading to a free 20-minute consultation. Nothing gets invented; gaps like credentials or prices are left for you to fill. It does not include recording videos, logo design or ad management. Read more about the white-glove service.

2. Dubsado: best for form-driven admin workflows

If your wellness practice runs on paperwork (questionnaire, contract, invoice, reminder, follow-up questionnaire) and you want each step to trigger the next, Dubsado is built for exactly that.

Dubsado homepage presenting its client workflow CRM for forms, contracts and invoices
Dubsado's homepage in October 2026.

Where it helps a wellness coach

Dubsado is a client workflow CRM with forms, contracts, invoices, a scheduler and automated workflows. A wellness coach can build a sequence where a discovery-call booking sends a lifestyle questionnaire, a completed questionnaire sends the agreement, and a signed agreement sends the first invoice. For coaches who sell corporate wellness days or retreat packages, the same forms, contracts and invoices cover multi-step paperwork.

Pricing as listed in October 2026

  • Starter: $35/month ($27.92/month billed annually), unlimited clients and projects, 1 active lead-capture form, no scheduler and no automations.
  • Premier: $55/month ($43.75/month billed annually), unlimited lead forms, scheduler and workflows.
  • 21-day trial with full Premier access, no credit card required. No Dubsado platform fee is stated; processor rates apply, and optional surcharging can pass card costs to the client.

Gaps for wellness work

Dubsado says itself that it does not include website building and is not an email marketing platform. It has no courses, client community, habits or check-ins, so once the intake is done, the program delivery has to happen somewhere else. The mobile app is a companion; full functionality needs the desktop web app. For a wellness coach, the Starter plan is a weak fit, because the automation that makes Dubsado worth it starts on Premier.

Pick it over Coachful if your offers are mostly one-off packages or corporate events, you never run multi-week programs with habits, and you want very granular paperwork automation more than client delivery.

3. Quenza: best for worksheets and activities between sessions

Quenza wins when the heart of your intake is reflective: a wellbeing wheel, a values exercise, a stress inventory, followed by worksheets the client fills in between sessions.

Quenza homepage presenting its client engagement tool with activities and worksheets
Quenza's homepage in October 2026.

What a wellness coach gets

Quenza is a client-engagement tool used by coaches and therapists. You build activities, worksheets and multi-step programs, send them to clients through a client app, and chat with them there. A wellness coach can send a sleep diary on Monday, a stress-trigger worksheet on Wednesday and a weekly reflection on Sunday, all as structured activities rather than loose PDFs.

Plans and limits (as listed in October 2026)

  • Spark: $25/month ($20 billed annually), 1 professional, 10 clients.
  • Growth: $50/month ($40 billed annually), 250 clients.
  • Impact: $125/month ($100 billed annually), 400 clients.
  • Collective: $160/month ($128 billed annually), up to 3 professionals, 500 clients.
  • Beyond: custom pricing for unlimited clients and professionals. A 30-day trial needs no credit card.

The catch

Quenza does not process client payments today; invoicing and digital products are marked as planned. An older profile also lists no scheduling and no video calls. That means a wellness coach using Quenza for intake still needs separate tools to book the discovery call, sign the agreement and take payment. It suits coaches who already have that admin sorted and want better engagement material.

Pick it over Coachful if you already have booking, contracts and payments handled elsewhere, you only need a worksheet and activity layer, and you have 10 or fewer clients and want the lowest monthly figure.

Which one looks like you?

Coachful builds your coaching website from a few answers, in a design chosen for your niche, with programs, booking and payments ready behind it.

4. Practice Better: best for practitioner-style charting

Practice Better suits the wellness coach who also works like a practitioner: charted session notes, telehealth sessions and a client portal, often alongside a nutrition or naturopathic credential.

Practice Better homepage presenting its practice management platform for wellness practitioners
Practice Better's homepage in October 2026.

How it handles intake

Practice Better is practice management for wellness and nutrition practitioners, covering scheduling, telehealth, charting, billing, programs and a client portal. Because charting sits next to scheduling and the portal, it is a natural fit if you write session notes like a clinician. If you work with licensed practitioners or hold a license yourself, check Practice Better's current compliance documentation directly rather than relying on any roundup.

Pricing as listed in October 2026

  • Sprout: $0/month, 3 clients, 1 practitioner, 100 MB storage.
  • Starter: $35/month ($25 billed annually), 10 clients, 1 GB.
  • Professional: $69/month ($59 billed annually), 300 clients, 5 GB.
  • Plus: $99/month ($89 billed annually), unlimited clients, 3 admins, unlimited storage.
  • Team: $155/month ($145 billed annually), 2 practitioners, up to 200 at +$50 each.
  • Trials: 30 days on Team, 14 days on other paid plans. Payments run through Practice Better Payments, powered by Stripe; rates are not listed.

Watch-outs

Evergreen programs, portal branding, a custom URL and SMS only start on Plus at $99/month, so a wellness coach who wants a branded program experience lands on the higher tier. An older profile notes no website builder or funnel pages, so the discovery-call page and lead magnet live elsewhere. Below Team, each account has one practitioner. Trustpilot showed 3.8 from 87 reviews when checked in October 2026.

Pick it over Coachful if you write clinical-style charts, you need a clinical record system for licensed work, or you want to test the market with three clients on the Sprout plan, which costs nothing.

5. HoneyBook: best for selling workshops to employers

Corporate wellness is a big slice of many wellness practices, and HoneyBook is strongest when the "client" is an HR team buying a lunch-and-learn series, not an individual filling in a health questionnaire.

HoneyBook homepage presenting its client management CRM for service businesses
HoneyBook's homepage in October 2026.

Fit for a wellness practice

HoneyBook is a client-management CRM for service businesses: proposals, contracts, invoices, a scheduler, lead forms and a client portal. A wellness coach can send an employer a proposal for a six-week stress workshop, have it signed and paid in one flow, then use a lead form to collect participant details. For pitching companies, our guide to software for corporate wellness coaches goes deeper.

What it costs (as listed in October 2026)

  • Starter: $29/month billed annually (the monthly figure is not shown on the page), unlimited clients and projects, 2 live lead forms.
  • Essentials: $59/month ($49 billed annually), up to 2 team members, 10 lead forms.
  • Premium: $129/month ($109 billed annually), unlimited team members and lead forms.
  • 30-day trial with no credit card. HoneyBook is the payment processor: its FAQ lists card fees of 2.7% + 10¢ while its plan comparison shows 2.9% + 25¢, so confirm before you rely on either. ACH is 1.5%.

Limits to know

HoneyBook has no coaching delivery: no programs, courses, habits, check-ins or community. The scheduler and automations are not on Starter. Payments must run through HoneyBook's own processing, and prices rose in February 2025, per an older profile. For individual wellness clients, the intake ends at the form and the real work moves elsewhere. Trustpilot showed 4.1 from 748 reviews in October 2026.

Pick it over Coachful if most of your revenue comes from employer contracts and proposals, you rarely coach individuals through multi-week programs, and you are happy with HoneyBook as your processor.

How do the five tools compare feature by feature?

Coachful is the only one of the five that covers intake, agreement, payment, program delivery and daily check-ins in one place. The matrix below uses each vendor's documented features as listed in October 2026.

ToolIntake formsContractsBookingPaymentsPrograms, habits, check-insWebsite and funnels

What does each tool cost at 5, 15 and 40 wellness clients?

At small client counts the gap is narrow; it widens once you add the tools each option is missing. Figures are monthly list prices as listed in October 2026, before card processing.

Tool5 clients15 clients40 clients
CoachfulLite: $29/month for up to 5 clientsSolo: $49/month for up to 20 clientsPro: $99/month with unlimited clients
Dubsado$55 Premier (or $35 Starter without scheduler)$55 Premier$55 Premier
Quenza$25 Spark$50 Growth$50 Growth
Practice Better$35 Starter$69 Professional$69 Professional ($99 Plus for branded portal)
HoneyBook$59 Essentials (scheduler included)$59 Essentials$59 Essentials

Remember what is not in those numbers. Dubsado, Quenza and HoneyBook have no program delivery with habits and check-ins, and none of the four other tools includes a website with funnels and email, so a wellness coach using them usually pays for at least one more tool.

What do three real-world wellness intake stacks cost per month?

Here are three example stacks for a solo wellness coach running a 12-week reset program with about 15 clients. They are illustrations built from listed prices, not quotes; add-on tools you would need for website and email are named but not priced, because the choice varies.

StackWhat it coversStill missingMonthly cost (example)
Coachful aloneIntake, agreement, booking, payments, program, habits, check-ins, community, website, emailNothing for this scenarioSolo: $49/month for up to 20 clients

Practice Better PlusIntake, charting, booking, telehealth, payments, branded portal, programsWebsite, funnels, email marketing, community$99 plus website and email tools

The Dubsado and Quenza stack also means two logins for you and two places for the client: one for paperwork, one for activities. That is the hidden cost wellness coaches feel most, because clients who are already stretched thin drop off when the first week asks them to set up two accounts.

How does a wellness coach set up intake in Coachful, step by step?

A full setup takes one focused afternoon, or you can ask the white-glove team to build the forms and funnel for you. Here is an example for a coach launching a 12-week "Sleep, Stress and Energy Reset."

A laptop showing a client overview with populated client profiles on a calm wooden desk

  1. Write the questions. Start from our health coaching intake form template or the intake form generator. Split them into a short pre-call set (goal, sleep, stress, the red-flag screen) and a longer onboarding questionnaire (meals, movement, caffeine, alcohol, routines, support at home).
  2. Add the short set to the discovery-call session type. Set availability, buffers and minimum notice, connect Google, Apple or Outlook calendar, and pick built-in video or an auto-created Zoom, Meet or Teams link.
  3. Put the scope line and red flags first. If someone answers yes to chest pain on exertion or a current eating disorder, your follow-up is a referral conversation, not a sales call.
  4. Build an application funnel for the program. Use one of the funnel templates, connect your Meta Pixel or GA4 if you advertise, and let applications land in your leads with the form as the source.
  5. Attach the coaching agreement. It goes out for e-signature before the first session and is stored on the client record. See coaching contracts for what to include.
  6. Set up payment. Connect Stripe Connect or Razorpay and offer a pay-in-full price plus a payment plan.
  7. Assign the long questionnaire as a form. Use the Forms & Worksheets resource so the client completes it in their branded app before session one.
  8. Turn the answers into the program. Build the 12 weeks from a program template or ask Michelle to draft it, then add habits (lights out by 10:30, a 10-minute walk after lunch) and morning and evening check-ins that echo the intake goals.
  9. Repeat the intake at week 12. Send the same core questions again so you and the client can compare sleep, stress and energy against the starting point.

What intake mistakes should wellness coaches avoid?

The biggest mistake is collecting medical detail you cannot act on and should not hold. Others follow close behind:

  • Asking for a full medical history. Ask about conditions that change your coaching or need clearance; leave diagnoses and lab values to the client's doctor.
  • No scope line. Without it, a client can reasonably think you are treating their insomnia or gut issue.
  • One 60-question form before the first call. A long pre-call form can put people off booking. Keep the pre-call set short and move depth to onboarding.
  • Answers nobody rereads. If the intake is a PDF in your inbox, it will not shape week six. Keep answers on the client record.
  • Skipping the re-intake. The end-of-program comparison is your best evidence of change and your best renewal conversation.
  • Copying an official screening form verbatim. Paraphrase screens like the PAR-Q+ and name the source in plain text.
  • Using a coaching tool for clinical records. If you hold a clinical license, keep that work in a clinical system.

Our list of coaching intake form questions has more examples of what to ask and what to leave out, and the ICF Code of Ethics is a useful reference for confidentiality and referral duties.

Which intake form software should a wellness coach choose?

Choose Coachful if you deliver programs and want intake answers to shape habits, check-ins and sessions under one login. Its intake questions on booking, application funnels, e-signed agreements and client records cover the full wellness journey, and the free white-glove build removes most of the setup.

Choose Dubsado if your practice is paperwork-heavy and program-light. Choose Quenza if you only need a worksheet layer and already handle admin elsewhere. Choose Practice Better if you chart like a practitioner or need a clinical system. Choose HoneyBook if employers, not individuals, are your main buyers. For more niche comparisons, see our roundups for holistic health coaches and health coaches.

Frequently asked questions

What should a wellness coach intake form include?

A goal, current sleep, stress, movement and eating patterns, readiness to change, support at home, and a short red-flag screen for conditions that need a doctor's clearance. Add a scope line saying you do not diagnose or treat, plus consent for how you store answers.

Do wellness coaches need HIPAA-compliant intake software?

Most coaches who are not health plans, clearinghouses or providers billing electronically are not HIPAA covered entities. If you are a licensed clinician or work inside a clinic, clinical records belong in a clinical system. Check with an attorney if you are unsure; Coachful is not HIPAA-certified.

Can I use a free form builder for wellness intake?

You can, but it only collects answers. You still need booking, an agreement, payment and a way to reuse the answers during the program, which usually means three more tools and copying answers by hand.

Which tool is best if I mainly sell corporate wellness workshops?

HoneyBook is strong for proposals and contracts with employers. If you also coach individuals after the workshop, Coachful handles group programs, community and individual intake in one place.

Is Practice Better or Coachful better for wellness coaches?

Practice Better suits practitioners who chart clinically and want telehealth with notes. Coachful suits coaches who deliver programs with habits, check-ins, courses, a community and a website under one login.

How long should a wellness intake form be?

Keep the pre-call form to roughly 8 to 12 questions and move the detailed lifestyle questionnaire to onboarding, after the agreement is signed. That keeps the first step light and still gives you depth before session one.

What is Coachful's free white-glove service?

From Create, you describe what you want built, such as an intake form, a discovery-call funnel, a program or a website, and attach references. The Coachful team builds it in your account, usually within about 24 hours, without inventing credentials, testimonials or prices.

Can I export intake answers if I leave Coachful?

Yes. A full client data export comes as a ZIP with your roster, notes, check-ins, goals, habits, sessions and form responses.

Should I repeat the intake at the end of a program?

Yes. Sending the same core questions at the end lets you and the client compare sleep, stress and energy with the starting point, which makes progress visible and opens the renewal conversation.

When should a wellness coach refer a client out?

Refer when answers point to chest pain or fainting with exertion, an eating disorder, pregnancy complications, recent surgery, medication changes or thoughts of self-harm. Coaching can continue alongside care once a qualified professional is involved.
