Wellness Coach vs Health Coach: Is There a Real Difference?

Wellness Coach vs Health Coach: Is There a Real Difference?

Wellness coach vs health coach: in the US they are largely the same profession under two names. The national certification body credentials them together as health and wellness coaches, the research literature studies them as one field, and neither title is licensed. The real difference is emphasis. "Health coach" usually signals work on specific health behaviours, often alongside a care team; "wellness coach" usually signals a broader focus on stress, sleep, energy and balance, often in private practice or workplace programmes.

Below is a side-by-side comparison, what each one does and may not do, what the credentials mean, how both compare with dietitians, life coaches and personal trainers, and, for coaches, which title to use.

The settings and focus rows describe common usage, not rules. Plenty of wellness coaches work with chronic conditions and plenty of health coaches work on stress.

Why the two titles overlap so much

Three things tie them together:

  • One national board. The National Board for Health & Wellness Coaching certifies "Board-Certified Health & Wellness Coaches (NBC-HWCs)" and uses the combined term throughout its scope of practice (NBHWC scope of practice). There is no separate national wellness coach credential from the same body.
  • One research field. A systematic review of 284 articles defined "health and wellness coaching" as a single intervention: patient-centred (in 86% of articles), with goals the patient sets (71%), self-discovery rather than passive advice (63%), accountability (86%) and some education (91%), usually within an ongoing relationship with a trained coach (78%) (Wolever and colleagues, 2013).
  • One set of billing codes. The CPT codes 0591T to 0593T cover health and wellness coaching, and on July 14, 2026 CMS released a proposed rule on Medicare coverage for them; the proposal was not final at the time of writing (NBHWC summary). See can a health coach bill insurance for the detail.

So the choice between titles is mostly about who you want to reach and how they describe their problem.

What a health coach does

A health coach helps people change behaviours that affect a health outcome. Typical work: building an eating routine that fits a busy week, getting from no exercise to three walks a week, improving sleep, keeping up with a treatment plan their doctor set. Many work in or with clinics, where the coach handles the behaviour change and the clinician handles diagnosis and treatment.

The NBHWC's 2025 survey of its certified coaches gives a sense of where the work happens: of 2,328 respondents, 41% worked mainly in private practice, 22% in healthcare, 10% at for-profit digital health companies and 7% in corporate wellness; 87% delivered sessions by video (NBHWC Annual Survey 2025). The survey covers certified coaches only, not everyone using either title.

What a wellness coach does

A wellness coach usually starts wider. The first conversation might cover energy, stress, sleep, movement, relationships and work, often using a tool like a wheel of life, before narrowing to one or two areas. The methods are the same as health coaching: client-set goals, small experiments, accountability. The emphasis is prevention and day-to-day wellbeing rather than a particular condition.

Wellness coaches are common in employer wellbeing programmes, in gyms and wellness centres, and among coaches who also teach yoga, breathwork or mindfulness. Some are burnout or stress specialists; see burnout coaching and stress management coaching.

What neither may do

Both titles share the same scope limits. The NBHWC's scope of practice says coaches on their own do not diagnose, interpret medical data, prescribe or de-prescribe, recommend supplements, provide nutrition consultation or create meal plans, provide exercise prescription or instruction, or provide psychological therapeutic interventions. It describes the coach's role as "accountability partner, not director." A coach who also holds another licence, such as a registered dietitian or nurse, may work within that licence if the dual role is disclosed and kept separate (NBHWC).

State laws on nutrition advice vary and can be strict; can anyone be a nutrition coach covers them.

Does health and wellness coaching work?

The evidence is promising, with gaps. A compendium of the health and wellness coaching literature identified 219 articles meeting its criteria, 150 of them data-based, and concluded that coaching is "a promising intervention for chronic diseases though further research is needed in most categories" (Sforzo and colleagues, 2018). In weight management, a meta-analysis of 14 studies found that health coaching combined with self-monitoring apps produced about 2.15 kg more weight loss than comparison groups, but no significant difference in BMI or blood pressure (Chew and colleagues, 2023). There is no evidence that one title gets better results than the other. Our guide to the benefits of a health coach matches nine benefits to the research.

Which one should you hire?

  • You have a diagnosis or a specific health target (blood sugar, weight, recovery after an illness): look for a health coach, ideally NBC-HWC, who is comfortable working alongside your doctor.
  • You feel depleted, stressed or out of balance but are broadly well: a wellness coach is a natural fit.
  • You need a meal plan for a medical condition: see a registered dietitian, and add a coach for the habits if you want one.
  • You are struggling with mood, anxiety or eating in a way that worries you: start with a doctor or licensed therapist.

Either way, check training and the NBC-HWC credential, ask how they work with clinicians, and ask what a typical package looks like. Questions to ask a health coach has a full list, and what is health and wellness coaching covers the basics. To browse coaches, Coachful's Health & Fitness directory is open to anyone.

What health and wellness coaching costs

Coachful's 2026 Coaching Business Report treats health and wellness as one niche. US figures: $75 to $200 an hour (PayScale and practitioner reports); a 4-week reset of four sessions at $400 to $800; a 3-month programme of 8 to 10 sessions with weekly check-ins at $900 to $2,000; ongoing monthly coaching at $300 to $700 (practitioner reports). For coaches: which title should you use?

Use the words your clients use. If they come to you about blood sugar, weight or a doctor's advice, "health coach" fits. If they come about burnout, stress and energy, "wellness coach" may land better. Many coaches use "health and wellness coach" and let the niche do the work: "wellness coach for nurses on night shifts" says more than either title alone. Credentials and scope are identical, so the decision is marketing, not law.

Whatever you call yourself, the practice runs on the same things: programmes with habits and check-ins, intake forms, sessions and a client app. Coachful includes all of them in your membership; see Coachful for health coaches, how to become a health coach and health coach salary.

Frequently asked questions

Is a wellness coach the same as a health coach?

Largely, yes. Both are unlicensed titles in the US, both are certified by the same national board as health and wellness coaches, and research treats them as one field. The difference is usually focus: health coaches lean toward specific health behaviours, wellness coaches toward overall wellbeing.

Which is better, a health coach or a wellness coach?

Neither is better in general. Choose a health coach for a specific health goal or a condition you manage with a doctor, and a wellness coach for stress, energy and balance. Check training and the NBC-HWC credential either way.

Can a wellness coach give nutrition advice?

General, publicly available healthy-eating information, yes, depending on state law. Individual meal plans and medical nutrition therapy, no, unless the coach also holds a licence such as registered dietitian. The NBHWC scope of practice excludes creating meal plans.

Do health coaches need to be certified?

No law requires it, but certification signals training. The NBC-HWC is the national credential for both health and wellness coaches, and many employers and clinics ask for it.

Is a wellness coach the same as a life coach?

No. A life coach can work on any life goal, while a wellness coach focuses on health and wellbeing habits. The methods overlap, and some coaches offer both.

How much does a wellness coach cost?

Coachful's 2026 Coaching Business Report puts health and wellness coaching at $75 to $200 an hour in the US, with 3-month programmes of 8 to 10 sessions at $900 to $2,000. Employer wellness programmes sometimes cover it.

Will Coachful build my wellness or health coaching website for free?

Yes. On every Coachful plan, the team builds your website for free, usually within about 24 hours, inside your own account so you can edit it afterwards. Request it from Create or the build-it-for-you card on the Website page.
