How to Start a Gut Health Coaching Business: A 90-Day Plan

How to Start a Gut Health Coaching Business: A 90-Day Plan

Here is how to start a gut health coaching business in nine steps: draw a clear scope line (you coach habits, a clinician diagnoses and treats), pick one client you can describe in a sentence, build two or three packages around a 12-week change window, price them from the hours they really take, set up a sole proprietorship or LLC with insurance and a signed coaching agreement, put booking, payments, programs and check-ins on one platform, win your first 10 clients through warm contacts and referral partners, deliver with daily habits and weekly check-ins, then follow a 30/60/90-day launch plan. Working part time, the plan below aims to have you taking paid clients within 30 days and running a first group cohort by day 90.

This guide assumes you have already decided to coach and have (or are finishing) a credential. If you are still choosing a program, read our comparison of health coach certifications first. What follows is about the business: the offer, the money, the paperwork, the tools and the first clients.

What are the steps at a glance?

The checklist below shows each step, a realistic time budget for a solo coach working part time, and the typical cost. Where a cost depends on your state or your quotes, it says so instead of guessing.

1. Write your scope and referral policy2 to 3 hours$0
2. Define one client and one problem1 week$0
3. Build 2 or 3 packages3 to 5 days$0
4. Price from hours and capacity1 afternoon$0

Is gut health coaching a viable niche?

Yes, if you position it as habit and lifestyle support that works alongside medical care rather than as a cure. Digestive complaints are common, persistent and frustrating, which is exactly the kind of problem people pay a coach to help them stay consistent with.

Demand you can point to

The US National Institute of Diabetes and Digestive and Kidney Diseases (NIDDK) estimates that 60 to 70 million people in the United States are affected by digestive diseases. For irritable bowel syndrome alone, NIDDK says studies suggest about 12 percent of people in the United States have IBS, women are up to two times more likely than men to develop it, and people under 50 are more likely to develop it than people over 50. That gives you a large, well-defined group of adults who live with symptoms day to day, often after a diagnosis that comes with a list of lifestyle changes and very little support for actually making them.

Who actually pays

  • Diagnosed and stuck. Adults with an IBS diagnosis (or another condition their doctor is managing) who were told to change how they eat, move, sleep and manage stress, and want someone to help them do it week after week.
  • Cleared but uncomfortable. People whose doctor has ruled out anything serious but who still deal with bloating, irregularity or food anxiety, and want structure.
  • Prevention-minded. Busy professionals and parents who want more fiber, fewer takeaway meals and better routines, with gut comfort as the motivating outcome.

The first two groups usually pay more and stay longer because the problem is felt every day. The third is easier to reach with content but more price sensitive. Our guide on whether a health coach is worth it covers how buyers weigh the decision.

Thinking of launching your coaching business?

Answer a few questions and Coachful builds it around you: your website, programs, booking and payments. Ready in minutes.

Build your platform

Where coaching ends and medicine begins

Scope line: gut health coaching is not medical treatment. You do not diagnose, order or interpret tests, treat conditions such as IBS, SIBO, celiac disease or inflammatory bowel disease, or tell clients to change or stop medication. You help a client follow the plan their clinician or registered dietitian has set, and you build the habits around it: meal rhythm, fiber and fluid, movement, sleep, stress practice and planning.

Write a short referral policy before your first client. Ask in your intake whether the client has seen a doctor about their symptoms, and refer back to a clinician before continuing when a client reports warning signs such as blood in the stool, black stools, unexplained weight loss, persistent vomiting, difficulty swallowing, fever, symptoms that wake them at night, or new symptoms later in life. If a client's clinician recommends an elimination diet such as low-FODMAP, the dietitian runs the protocol; you help with shopping, prep and sticking to it. Mood and eating patterns also come up often in this niche, so keep our guide to mental-health warning signs and referral close.

Some US states restrict who may give individualized nutrition advice, and rules differ by state. The SBA notes that license requirements depend on your business activity and location and tells owners to check state and local government websites. Check your state's dietetics licensure rules, and ask an attorney if your offer includes meal plans for people with diagnosed conditions.

Who is your client and what do you sell them?

Pick one person and one problem, then build offers around a change window long enough to form habits. A useful sentence: "I help [who] go from [daily struggle] to [specific, non-medical outcome] in [time], alongside their doctor's plan."

Example: "I help women in their 30s and 40s with an IBS diagnosis go from guessing what to eat at work to a weekly meal rhythm and stress routine they can keep, in 12 weeks." It names who, the daily struggle, a habit outcome rather than a cure, and a time frame.

Three example packages

  1. Calm Gut 1:1 (12 weeks). Six 45-minute calls every other week, a short intake call, morning and evening check-ins, a symptom, meal and stress journal, weekly habit goals, and message support on weekdays. This is your flagship.
  2. Gut Reset Group (8 weeks). A cohort of 8 to 15 people with one weekly group call, a shared community space, a weekly lesson and the same daily check-ins. Lower price, more clients per hour, and a natural next step for people who are not ready for 1:1.
  3. Steady Gut Membership (monthly). For graduates: one group Q&A a month, a monthly check-in, recipes and meal-prep plans, and continued habit tracking. This is what keeps revenue steady after the first program ends.

Build each package so it is clear what is included, how long it runs, how to reach you, and what happens when it ends. Our guide to creating a health coaching program clients can keep walks through the week-by-week structure, and the coaching packages page shows how packages are set up in Coachful.

How should you price gut health coaching?

Price from the hours a package really takes and the income you need, then check it against what your client can afford. Do not copy a number from social media; work it out.

Start with the hours. For the 12-week 1:1 package above: six 45-minute calls (4.5 hours), plus about 15 minutes a week reviewing check-ins and replying to messages (3 hours), plus about 2 hours of onboarding and admin. That is roughly 9.5 hours per client. Divide your package price by that number to get your effective hourly rate, and make sure it covers your taxes, software and unpaid marketing time.

The table uses illustrative numbers only. They are not market rates; replace them with your own prices and capacity.

Package (example)Example priceHours per clientEffective hourlyClients per quarterQuarterly gross
Calm Gut 1:1, 12 weeks$1,2009.5About $1268$9,600
Gut Reset Group, 8 weeks$400About 1.5 (12 hours of calls and prep shared across 12 people, plus check-in review)About $26712$4,800
Steady Gut Membership$60/monthAbout 0.5 per monthAbout $12015 members x 3 months$2,700
Total (example)$17,100

Two adjustments before you publish prices. First, subtract card processing from every sale (check your processor's pricing page for your country). Second, set money aside for tax: the IRS says self-employment tax is 15.3% (12.4% Social Security plus 2.9% Medicare) on net earnings, on top of income tax. Payment plans help with a 12-week package: three monthly payments make a larger price easier to say yes to. For pay data across health coaching, see our health coach salary guide.

What legal setup does a gut health coach need?

At minimum: a business structure, a tax ID if you need one, professional liability insurance, a written coaching agreement that states your scope, and a plan for client data. None of this is complicated, but the details vary by state and country, so check with an accountant or attorney where your situation is unusual.

Entity and tax ID

The SBA describes a sole proprietorship as automatic if you do business without registering another structure, and "a good choice for low-risk businesses and owners who want to test their business idea," with the trade-off that you can be held personally liable for business debts. An LLC protects owners from personal liability "in most instances," and profits can pass through without corporate tax, though members pay self-employment tax (SBA: choose a business structure). Many health coaches start as sole proprietors and move to an LLC once they have steady clients.

If you form an LLC, form it with your state first, then get an EIN. The IRS issues an EIN online "in minutes for free" and says "You never have to pay a fee for an EIN" (IRS: get an EIN). Sole proprietors report business income on Schedule C.

Taxes you will owe

  • You must pay self-employment tax and file Schedule SE if net earnings from self-employment are $400 or more (IRS).
  • You generally must make estimated payments if you expect to owe $1,000 or more when you file (IRS: estimated taxes).
  • The remaining 2026 estimated payment is due January 15, 2027; it is not required if you file your 2026 return by February 1, 2027 and pay the full balance (Form 1040-ES (2026)).

Insurance

Professional liability insurance is the one to prioritise. The SBA says it is for "Businesses that provide services to customers" and "protects against financial loss as a result of malpractice, errors, and negligence." It also notes that an LLC can protect personal property from lawsuits, "However, that protection has limits" (SBA: get business insurance). When you request quotes, describe your work accurately as wellness or health coaching, mention nutrition-related habit support, and confirm the policy covers online sessions with clients in other states or countries.

Coaching agreement

Your agreement should state what coaching is and is not, that the client remains under the care of their own clinician, your referral policy, session and cancellation terms, payment and refund terms, confidentiality and its limits, and how you store data. Send it before the first call and get it signed. In Coachful, coaching agreements are signed electronically before the first session and stored on the client record; see coaching contracts.

Coachful Agreements tab with an empty state and a drop area for a signed PDF
The Agreements tab in Coachful before any agreements exist, with a drop area for a signed PDF.

Client data, testimonials and email

  • Health data. Gut clients will tell you about symptoms, bowel habits and medications. Collect only what you need for coaching, explain how you store it, and use a platform that lets you export or delete it. Coachful is not HIPAA-certified and does not sign a BAA, so if you coach inside a medical practice that requires one, run that work in the practice's own compliant system.
  • Testimonials. The FTC's Endorsement Guides say endorsements "must be honest and not misleading," and testimonials showing exceptional results need proof that they are what people generally achieve, or the ad must make clear the generally expected results (FTC). Since October 21, 2024 the Consumer Reviews and Testimonials Rule also prohibits fake reviews. "My bloating is gone" from one client is not a typical result; present it with context or not at all.
  • Email. Under CAN-SPAM, commercial email needs accurate headers, honest subject lines, a valid postal address and a clear opt-out honored within 10 business days (FTC compliance guide).

What tech stack does a gut health coach need?

You need booking, payments, agreements, a way to deliver a program with daily habits and check-ins, a place for group members to talk, a website and email. You can stitch that together from four or five tools, or run it on one platform with one login for you and one app for your clients. We make Coachful, and this is the setup we recommend for a new gut health practice.

What Coachful is: a coaching platform where one subscription covers booking, payments, contracts, program delivery with habits and check-ins, courses, a community, a website with funnels and email, and an AI assistant. Who it is for: coaches who deliver programs rather than single sessions, solo coaches growing into a small team, and coaches replacing four to six tools. What it does not do: it is not HIPAA-certified, it is not built for clinical records or insurance billing, it does not take PayPal, it has no permanent free plan, and it does not sync with Garmin, Fitbit, Whoop or Oura. How to start: begin with a free trial, then ask the team to build your site, funnel or program for you through the free white-glove service.

What each part of Coachful replaces

  • Booking app: booking pages with availability rules, buffers, minimum notice and time-zone detection; two-way Google, Apple/iCloud and Outlook sync; built-in video or auto-created Zoom, Meet or Teams links; intake questions on booking.
  • Payment and invoicing tool: your own Stripe Connect or Razorpay account for one-time payments, subscriptions, payment plans, coupons, refunds, Apple Pay and Google Pay, with an invoice per transaction. Coachful charges a 0% platform fee (since August 2026); you pay only Stripe or Razorpay processing.
  • E-signature tool: coaching agreements signed before the first call and stored on the client record.
  • Habit and journaling apps: multi-week programs with weekly goals, daily tasks, habits, morning and evening check-ins, goals with progress bars and streaks.
  • Food and meal-planning apps: the Fitness & Nutrition pack adds nutrition targets up to calories and macros, meal plans, recipes, food lookup and barcode scanning, and weekly check-ins.
  • Course and community tools: courses with drip and completion tracking, a community feed, group chat, direct messages and cohort groups created automatically when someone enrolls in a program.
  • Website builder and email tool: a section-based site with blog and custom domain, funnels, email lists, broadcasts and multi-step sequences.

The nutrition tools come from the Fitness & Nutrition pack; the rest is included in your Coachful membership. See the Coachful page for health coaches for a niche overview.

Which one looks like you?

Coachful builds your coaching website from a few answers, in a design chosen for your niche, with programs, booking and payments ready behind it.

Build your platform

The free white-glove build

Why Coachful stands out: a free white-glove service. From Create > We'll build it for you · FREE, you describe what you want built (a website, funnel, sales page, program, course outline and written material, email sequence, intake form, lead quiz and more), attach up to 8 reference links and up to 10 files such as your logo, photos and existing copy, and the Coachful team builds it inside your own account, usually within about 24 hours. Nothing gets invented: no fake credentials, testimonials or prices, and gaps are left for you to fill. It does not include recording videos, logo design, ad management or ongoing marketing.

One recent build: a menopause and midlife health coach starting from nothing got a five-page site in sage and cream with every button leading to a free 20-minute consultation. For a gut health coach, a similar request might be a site, a 12-week program outline and a "5-day gut-friendly meal prep" lead magnet funnel. Read more in our post on the free white-glove service.

How do you set up your website, booking and payments?

Build a small site with one goal (book a consultation), connect booking to your calendar, and make sure every package can be paid for online before you start marketing. Five pages are enough: Home, About, Work With Me, Results (when you have honest ones) and Contact, plus a privacy policy.

Your homepage should say who you help, the outcome in plain words, your scope line in one sentence ("I work alongside your doctor or dietitian, not instead of them"), and one button. Our health coach website guide lists what to include on each page. Coachful has 30 designed homepage templates; the wellness ones suit a calm, food-focused brand.

The short walkthrough below shows the Coachful website builder.

Booking. Create two session types: a free 20-minute consultation with intake questions, and paid package sessions that draw from session credits. Add a buffer between calls and a minimum notice period so clients cannot book you at 7 a.m. tomorrow. Turn on reminders; SMS (24 hours) and WhatsApp (1 hour) reminders are available once messaging is enabled.

Payments. Connect your Stripe account, then create each package as an offer: the 1:1 program as a one-time price plus a three-payment plan, the group as a one-time price, and the membership as a monthly subscription. Add Stripe Tax if you sell to places that require it, and check with an accountant about sales tax on digital services where you live.

How do you get your first 10 gut health clients?

Start with people who already know you and with professionals who see your ideal client every week but cannot give them weekly habit support. Content builds slowly; conversations convert now.

Channels that fit this niche

  1. Warm network. Tell 50 people what you do and who it is for. Many will know someone with IBS or chronic bloating.
  2. Referral partners. Registered dietitians who see clients monthly, personal trainers, yoga and Pilates teachers, pelvic floor physiotherapists and pharmacists all meet people who need consistency between appointments. Offer to support their plan, not replace it.
  3. A small lead magnet. A five-day "gut-friendly meal prep" email series or a fiber-and-fluid habit tracker, delivered through a funnel, gives people a low-risk first step.
  4. Short educational content. Practical posts about meal rhythm, packing a work lunch, eating out, or what to ask your doctor. Avoid cure language such as "heal your gut" or "fix IBS."
  5. Talks. Workplace wellness sessions, libraries and community centers often welcome a 30-minute talk on habits for digestive comfort.

Scripts you can adapt

Warm message: "Hi Sam, I've just opened my gut health coaching practice. I work with people who have digestive issues their doctor is already aware of and want help building a meal and stress routine that sticks. If anyone comes to mind, I'd be grateful for an introduction. I'm taking five founding clients this month."

Referral partner email: "Hi Dr. Patel, I'm a certified health coach working with adults who have IBS and other digestive concerns. I don't diagnose or change treatment plans; I help clients follow the plan you or their dietitian set, with weekly check-ins and daily habit tracking. I refer back immediately if a client reports warning signs. Could I send you a one-page summary of how I work?"

Post call to action: "If you've been told to 'eat more fiber and manage stress' and nobody showed you how, book a 20-minute consultation. Link in bio."

On the consultation itself, use a structured script; our health coaching discovery call script has five phases with a full example. Before the call, send a short intake form; the health coaching intake form template includes the medical-history and referral questions this niche needs.

How do you deliver results and keep clients?

Deliver through small daily actions and frequent light check-ins, not through long calls. Gut habits change slowly, and what keeps a client going is seeing that they did the thing, today, and that you noticed.

Build the program around habits

A 12-week Calm Gut program might run like this: weeks 1 and 2 on meal rhythm and a symptom, meal and stress journal; weeks 3 to 5 on fiber and fluid steps agreed with the client's clinician or dietitian; weeks 6 to 8 on movement after meals, sleep and a daily breathing practice; weeks 9 to 12 on eating out, travel, flare plans and a maintenance routine. Each week gets one goal and two or three daily habits.

Check-ins that take two minutes

Set a morning check-in (sleep, energy, plan for the day) and an evening check-in (comfort rating from 0 to 10, stress, which habits got done, one note). You review the pattern weekly rather than reacting to every bad day. A sudden change, or any warning sign, triggers a message and possibly a referral.

This walkthrough shows how check-ins are set up in Coachful.

In Coachful, the client record pulls goals, check-ins, habits, notes, intake answers, payments and sessions together, and marks each client as thriving, active, quiet or at risk, so you can see who has gone silent before they quietly drop out. Michelle, the built-in AI assistant, can brief you before each session from that live data.

Community and retention

Group members do better when they can see that others struggle with the same meals and social situations. When someone enrolls in the Gut Reset Group, Coachful creates the cohort group automatically, with a feed, group chat and video events for the weekly call.

Plan the end of every program from day one. In week 10, invite 1:1 clients to the membership; in the last group call, show members what the next month looks like. Monthly recipes, a monthly check-in and a group Q&A give graduates a reason to stay without much extra work for you.

What does a 30/60/90-day launch plan look like?

Spend the first 30 days getting legal and ready to take money, the next 30 winning founding clients, and the last 30 running your first group and building repeatable marketing.

WindowFocusDone when
Days 1 to 30Scope and referral policy, client sentence, two packages, pricing, entity and EIN, insurance quotes, coaching agreement, platform and five-page site, booking and payments liveA stranger could find your site, book a consultation, sign the agreement and pay

What mistakes do new gut health coaches make?

  • Promising a cure. "Heal your gut in 30 days" invites the wrong clients, scope problems and FTC trouble. Sell habits and support.
  • Acting as the clinician. Recommending tests, supplements for a condition, or elimination diets without a clinician's plan puts clients and your insurance at risk.
  • No referral policy. Decide in advance what you do when a client reports a warning sign, and put it in your agreement.
  • Too broad a niche. "Gut health for everyone" is hard to market. "IBS support for working parents" is easy to refer to.
  • Selling single sessions. One call rarely changes a digestive routine. Sell 8 to 12 week packages.
  • Only coaching on calls. Without daily check-ins, you learn about a bad fortnight two weeks late.
  • Five tools and five logins. Each extra app is another thing for a client to ignore and for you to pay.
  • Using one client's story as typical. Present results honestly and with context.

Frequently asked questions

Do I need a certification to start a gut health coaching business?

Coaching itself is generally not licensed, but a recognized health coaching credential helps with clients, referral partners and insurers. Some states restrict individualized nutrition advice, so check your state's rules. Our certification comparison covers the main options.

Can a gut health coach work with clients who have IBS?

Yes, as long as you coach habits around the plan their clinician or dietitian has set and do not diagnose or treat. Ask about medical care in your intake, and refer back when a client reports warning signs.

Can I recommend supplements or elimination diets?

Stay within your credential and your state's rules. Recommending supplements or diets to treat a condition is outside coaching scope. If a clinician recommends a low-FODMAP or other elimination diet, the dietitian runs it and you support consistency.

How long should a gut health coaching program be?

Most habit change needs weeks, not days, so 8 to 12 weeks is a practical core program. Offer a monthly membership afterwards so clients can keep their routines with lighter support.

Should I form an LLC before my first client?

Not necessarily. The SBA describes a sole proprietorship as a good choice for testing a business idea, while an LLC protects personal assets in most instances. Get professional liability insurance either way, and check with an accountant or attorney.

Is Coachful HIPAA-compliant?

No. Coachful is not HIPAA-certified and does not sign a BAA, and it is not built for clinical record-keeping or insurance billing. If you coach inside a medical practice that needs a BAA, use that practice's compliant system for that work.

What is Coachful's free white-glove service?

From Create > We'll build it for you, you describe a website, funnel, program, email sequence or other asset, attach up to 8 links and 10 files, and the Coachful team builds it inside your account, usually within about 24 hours. It invents nothing, and it does not include video recording, logo design or ad management. Details are in our white-glove service post.

Can clients log meals and food in Coachful?

Yes, with the Fitness & Nutrition pack, which adds nutrition targets up to calories and macros, meal plans, recipes, food lookup and barcode scanning. Habits and morning and evening check-ins are part of programs for every client.

How many clients do I need to replace a salary?

Work backwards from your target. Divide the annual income you need (plus tax and costs) by your package price to find how many packages a year you must sell, then check that your weekly hours can hold them. The pricing table above shows the method with example numbers.

Where do gut health coaches find referral partners?

Dietitians, personal trainers, yoga and Pilates teachers, pelvic floor physiotherapists and pharmacists all see people who need support between appointments. Lead with your scope and referral policy; partners refer to coaches who make their job easier and stay in their lane.
