How to Start a Health Coaching Business in 2026

How to Start a Health Coaching Business in 2026

If you want to know how to start a health coaching business, here is the short version: choose one specific client and health goal, package your coaching into two or three programs with clear prices, set up the legal basics (business structure, EIN, insurance, a coaching agreement with a scope clause), put booking, payments and program delivery in one place, then sign your first 10 clients through people and partners who already trust you. Most coaches can get from decision to first paying client in 30 to 60 days if they keep the setup lean.

This guide assumes you have already decided to coach and want to build the business around it. If you are still choosing a credential, read our comparison of health coach certifications first. Everything below is about turning that skill into a practice that pays, keeps clients and stays inside the law.

What are the steps to start a health coaching business?

There are nine steps, and the order matters: decide who you serve before you build anything, and handle the legal setup before you take money. The table shows realistic time and the typical cost of each step.

For a line-by-line budget across all coaching niches, our guide to what it costs to start a coaching business goes deeper. The rest of this article works through each step for health coaching specifically.

Is health coaching a viable business niche?

Yes, but it is crowded, so a general "I help people get healthy" offer struggles. Coachful's coaching rates research, which draws on PayScale and coach community reports, describes health and wellness as "the most crowded end of the profession, and the one competing hardest against apps and subscriptions." That is the honest starting point: your competitor is not only other coaches but every free tracking app and low-cost subscription.

The coaches who build a viable practice usually win on specificity and accountability, two things an app does poorly. The same research flags women's health as underserved and growing, and points out that a background in health, fitness or nutrition credentials is a natural fit for this niche.

Who actually pays for health coaching?

  • Individuals paying out of pocket. The largest group for a new coach. They pay for a specific outcome they have failed to reach alone: steady energy after a diagnosis, sleep that holds up through a demanding job, a sustainable way to eat during perimenopause.
  • Employers and wellness programs. Workshops, lunch-and-learns and group programs for staff. Longer sales cycles, larger invoices.
  • Referral partners. Personal trainers, physical therapists, chiropractors, yoga studios and some medical practices refer clients who need behaviour support between appointments.
  • Gyms and studios. Some will run a paid habits program for members and split the revenue with you.

If you want the client-side view of where coaching helps and where the evidence is thin, read Is a Health Coach Worth It?. Knowing the evidence by goal makes your sales conversations more honest and your positioning sharper.

What is your scope, and when do you refer?

Health coaching is not medical treatment, diagnosis or medical nutrition therapy. A health coach helps a client set goals, build habits and stay accountable to a plan, often one their doctor or dietitian has already set. Write this into your website, your intake form and your coaching agreement.

Refer out when a client describes symptoms that need assessment, wants a diet for a medical condition you are not licensed to prescribe, shows signs of disordered eating, or raises mental-health concerns beyond coaching. Our guides to mental-health warning signs in coaching and to working with chronic-condition clients cover the recognition and referral steps. Some states also regulate who can give individualized nutrition advice, so check your state's licensing rules before you write meal plans.

Thinking of launching your coaching business?

Answer a few questions and Coachful builds it around you: your website, programs, booking and payments. Ready in minutes.

Build your platform

How do you define your ideal client and offer?

Write one sentence: "I help [specific person] achieve [specific outcome] without [the thing they hate] in [timeframe]." If you cannot fill every bracket, your niche is still too broad.

Some examples that work:

  • "I help women in perimenopause rebuild steady energy and sleep in 12 weeks without restrictive dieting."
  • "I help shift workers in healthcare build a sleep and meal routine that survives night rotations."
  • "I help adults newly told they are prediabetic turn their doctor's advice into daily habits that stick."

Then shape your offer around how change actually happens: slowly, through repeated small actions and regular check-ins. That is why most health coaching packages are programs, not single sessions. Here are three package shapes you can adapt.

Example package 1: the 4-week reset

Four 60-minute sessions, a habit tracker and a simple food and movement plan. Good as a first purchase for people who are unsure about coaching, and as a front door to a longer program.

Example package 2: the 12-week program

Eight to ten sessions over three months, weekly check-ins and a resource library. This is where most of the real behaviour change happens, and it is the package most health coaches should lead with.

Example package 3: ongoing monthly support

Two to four sessions a month plus messaging between sessions. Offered to clients who finish the 12-week program and want to keep the momentum. Recurring revenue keeps your income stable.

A small group version of the 12-week program is a fourth option once you have a few clients. Our guide on how to create a health coaching program walks through the week-by-week structure.

Coachful Offers page with three live demo offer cards with cover photos and prices
The Offers page with demo packages, each with its own cover photo, price and live status.

How should you price health coaching packages?

Price from your income goal backwards, not from what feels comfortable. Our coaching rates research, compiled from coach community reports, lists typical health and wellness package ranges of $400 to $800 for a 4-week reset, $900 to $2,000 for a 3-month program and $300 to $700 a month for ongoing support, with hourly rates of roughly $75 to $200 (PayScale and community data). For pay data beyond packages, see our health coach salary guide.

The table below is illustrative only. It picks a sample price inside each range and shows how many clients you would need to reach an example target of $4,000 a month in gross revenue.

Package (example price)Monthly revenue per clientClients needed for $4,000/monthSessions per month (approx.)
Ongoing monthly at $450$4509 active clients18 (2 per client)
12-week program at $1,200$400 (spread over 3 months)10 active clients30 (about 3 per client)
4-week reset at $600$600 (one-time)7 new clients every month28 (4 per client)
6-week group program at $250 per personAbout $2,670 per cohort, averaged per month16 people per cohort6 group calls per cohort

Three things the table makes obvious. First, the 4-week reset alone is a treadmill: you would need seven brand-new clients every month. Second, longer programs and monthly support give you the same revenue with far less selling. Third, a group program earns well per hour but needs an audience before it fills.

Before you settle on a number, subtract what comes off the top. Card processing fees come off every payment. In the US, self-employment tax is 15.3% (12.4% Social Security plus 2.9% Medicare), and you owe it once net earnings reach $400, according to the IRS self-employment tax page. Income tax sits on top of that. Many coaches set aside a fixed share of every payment; ask an accountant what share fits your situation.

What legal setup does a health coaching business need?

At minimum: a business structure, a tax ID, professional liability insurance, any local licenses, a written coaching agreement with a scope clause, and a clear plan for client data. Most of this takes days, not months. The guidance below is US-focused; rules vary by state and country, so check with an accountant or attorney.

Sole proprietorship or LLC?

The SBA describes a sole proprietorship as "a good choice for low-risk businesses and owners who want to test their business idea," but notes you can be held personally liable for business debts. An LLC protects owners from personal liability "in most instances," and LLC members still pay self-employment tax. Many health coaches start as sole proprietors and form an LLC once revenue is steady; others form the LLC first because they work with clients' health goals and want the separation. Sole proprietors report business income on Schedule C.

EIN and taxes

The IRS issues an EIN online "in minutes for free," and you never pay a fee for one. If you form an LLC, do that with your state before you apply. Once you are earning, plan for quarterly estimated tax payments. For the 2026 tax year the due dates are April 15, June 15 and September 15, 2026, and January 15, 2027, according to Form 1040-ES (2026).

Insurance

The SBA lists professional liability insurance for "businesses that provide services to customers," protecting against "malpractice, errors, and negligence." For a health coach, this is the policy that matters most. Get quotes from two or three insurers that write policies for coaches or wellness professionals, and confirm the policy covers online sessions and clients in other states if you plan to coach them. The SBA also notes that the liability protection an LLC gives you "has limits," which is why insurance still matters with an LLC.

Coaching agreement

Every client signs a written agreement before the first session. For health coaching it should include: what coaching is and is not (the scope line from earlier), that the client remains under the care of their own medical providers, your referral policy, session and rescheduling rules, payment terms and refunds, confidentiality and its limits, and how you store their data. Have an attorney review your template once; after that you reuse it.

Client data and HIPAA

Health coaches collect sensitive information on intake forms. Collect only what you need, store it in one system with access controls, and tell clients how you use it. If you plan to work inside a medical practice or take referrals that involve protected health information, ask an attorney whether HIPAA applies to you and which tools you would need. Coachful is not HIPAA-certified and does not sign a business associate agreement, so it suits independent coaching, not clinical record-keeping or insurance billing. Our health coaching intake form template shows what to ask without over-collecting.

Testimonials and marketing email

Health results are the claims most likely to mislead. The FTC's Endorsement Guides say endorsements "must be honest and not misleading," and a testimonial showing exceptional results needs proof that it reflects what people generally achieve, or the ad must make the generally expected results clear. The Consumer Reviews and Testimonials Rule, in effect since October 21, 2024, bans fake testimonials outright. For your newsletter, the CAN-SPAM Act requires a valid postal address, a clear opt-out and honoring opt-outs within 10 business days.

What tech stack does a health coaching business need?

You need six jobs covered: booking, payments, agreements, client records, program delivery between sessions, and a website that turns visitors into consult calls. You can assemble those from separate tools or run them from one platform. We make Coachful, so weigh our view accordingly, but this is why we built it as the home for a health coaching practice.

Coachful is a coaching platform for coaches who deliver programs, not just sessions. One subscription covers booking, payments, contracts, programs with habits and check-ins, courses, a community, a website with funnels and email, and an AI assistant, with one login for you and one branded app for your clients. It fits solo health coaches who are starting out, coaches growing into a small team, and coaches replacing four to six separate tools. It is not HIPAA-certified, does not support PayPal, does not sync with Garmin, Fitbit, Whoop or Oura, and has no permanent free plan. You start with a free trial, and the team will build your setup for you at no cost.

Here is what each part replaces:

  • Booking replaces a standalone scheduler: booking pages with availability rules, buffers, minimum notice and time-zone detection, two-way Google, Apple/iCloud and Outlook calendar sync, and built-in video or auto-created Zoom, Meet or Teams links. See scheduling software.
  • Payments replace a separate checkout and invoicing tool: one-time payments, subscriptions, payment plans, coupons and refunds through your own Stripe Connect or Razorpay account, with Apple Pay and Google Pay. Coachful charges a 0% platform fee, so you pay only Stripe or Razorpay processing. See coaching payment software.
  • Agreements replace an e-signature tool: coaching agreements go out for e-signature before the first call and are stored on the client record. See coaching contracts.
  • Client records replace a spreadsheet CRM: goals, check-ins, habits, notes, intake answers, tags, payments and sessions in one place, with a status of thriving, active, quiet or at risk.
  • Programs replace a habit app plus a course tool: multi-week programs with weekly goals, daily tasks, habits, morning and evening check-ins and streaks.
  • Website, funnels and email replace a site builder and an email platform: a section-based website with a blog and custom domain, lead-magnet and booking funnels, and email sequences with triggers and delays.

If you coach nutrition or movement, the Fitness & Nutrition pack adds nutrition targets up to calories and macros, meal plans, recipes, food lookup and barcode scanning, private progress photos and measurements, and weekly check-ins.

How does that compare with assembling separate tools?

Here is how the main options for health coaches compare on the jobs above, with vendor prices as listed in October 2026.

Why Coachful stands out

Why Coachful stands out: a free white-glove service. From Create > We'll build it for you (or the "Want us to build it for you?" card on the Website, Funnels and Programs pages), you describe what you want: a website, a funnel, an offer page, a program, a course outline, an email sequence, an intake form and more. You can attach up to 8 reference links and up to 10 files such as your logo, photos and existing copy. The Coachful team builds it inside your own account, usually within about 24 hours, and invents nothing: no fake credentials, testimonials or prices, and any gaps are left for you to fill. One recent build was for a menopause and midlife health coach starting from nothing, who got a five-page site in sage and cream where every button led to a free 20-minute consultation. The service does not cover recording videos, logo design, ad management or ongoing marketing. Read more about the free white-glove service.

How do you set up your website, booking and payments?

Your website has one job at launch: get the right person to book a consultation. Five pages are enough: Home, About, Work With Me, a results or approach page, and Contact, plus a privacy policy.

On the homepage, put your one-sentence positioning statement at the top, a short "is this you?" list, how the program works in three steps, your scope line, and one button: book a free consultation. Repeat that button down the page. Our health coach website guide covers what to include section by section, and Coachful for health coaches shows how the pieces fit together.

The Coachful website builder includes 30 designed homepage templates, a blog, a custom domain and SEO settings. This short walkthrough shows how the website builder works:

For booking, create two session types: a free 20 to 30-minute consultation with intake questions, and paid sessions for your packages. Set buffers between calls, a minimum notice period and reminders. Email reminders are standard, and once messaging is enabled Coachful adds SMS 24 hours before and WhatsApp one hour before a session.

For payments, connect your own Stripe account and set up each package as a product: the 4-week reset as a one-time payment, the 12-week program as a one-time payment or a payment plan, and ongoing support as a monthly subscription. Each transaction generates an invoice, and you can create manual invoices for employer workshops.

Build your platform

How do you get your first 10 health coaching clients?

Your first 10 clients will almost certainly come from people who already know you and from professionals who see your ideal client every week. Ads and content work later; at launch, direct conversations work faster.

Channel 1: your warm network

Make a list of 50 to 100 people you know. You are not asking them to buy; you are asking whether they know someone who fits your client sentence. A message that works:

"Hi Sam, I've just launched my health coaching practice, working with women in perimenopause who want their energy and sleep back without crash diets. Do you know anyone going through that who might want a free 20-minute call? No pressure either way."

Channel 2: referral partners

Personal trainers, physical therapists, chiropractors, massage therapists, yoga and Pilates studios, and some primary care practices all see clients who need help with habits between appointments. Offer a clear handoff and a promise to stay in your lane:

"I'm a health coach who works with clients on sleep, eating routines and daily movement habits once their provider has given them a plan. I don't diagnose or prescribe; I help people follow through. If you have clients who struggle between visits, I'd be glad to offer them a free consultation and send you a short update with their permission."

Channel 3: talks and workshops

A free 45-minute talk at a library, a community centre, a gym or an employer lunch-and-learn puts you in front of 10 to 30 of the right people at once. End with an invitation to book a consultation and a sign-up sheet for a free resource, such as a seven-day sleep reset guide.

Channel 4: one online community and one content platform

Pick one place your ideal client already gathers online (a local parents' group, a condition-specific forum, a professional network) and answer questions helpfully for a month. Pair it with one content channel where you post short, practical tips with a link to your consultation page. Our list of coaching communities for health coaches is useful for your own peer support too.

Channel 5: a free challenge funnel

A five-day challenge ("5 days to calmer evenings") collects emails, gives people a small win and ends with an offer to book a call. In Coachful you can build this as a lead-magnet funnel with an email sequence; the funnel templates give you a starting point.

Run every consult the same way: understand their situation, clarify what they want, explain how your program works, check fit and scope, then make a clear offer. Our health coaching discovery call script gives you the five phases and a full example.

How do you deliver programs that keep health coaching clients?

Clients stay when they see progress between sessions, not just during them. That means daily actions, regular check-ins and a coach who notices when they go quiet.

A 12-week program usually has a weekly theme, two or three daily habits, a short morning or evening check-in, and a session every week or two. In Coachful, you build this once as a program with weekly goals, daily tasks, habits and check-ins, then every client who enrols gets it in their branded app.

The client's Growing Habits view with demo habits and their progress toward 30 days.

Check-ins are the backbone. A morning check-in can ask about sleep and energy; an evening one about whether today's habits happened and what got in the way. Answers land on the client record, so you walk into each session already knowing what the week looked like.

This walkthrough shows how check-ins work from the coach's side:

Watch client status closely. Coachful marks each client as thriving, active, quiet or at risk, so you can message the quiet ones before they drift away. Our piece on client health score audits explains how to use those signals without over-reading them, and the habit tracking apps roundup compares the options.

Community adds accountability that one-to-one coaching cannot. When a client enrols in a program, Coachful creates a cohort group for it automatically, so people going through the same weeks can share wins and questions. Add a monthly live Q&A and you have a group layer that also prepares you for a group program later. See group coaching platforms for health coaches for more on that step.

Finally, plan the next step before the program ends. Around week 9 of a 12-week program, review progress with the client and offer ongoing monthly support. That one conversation is the difference between restarting your client search every quarter and building a stable practice.

What does a 30/60/90-day launch plan look like?

Here is a realistic plan for a coach launching part-time alongside a job. Compress it if you are full-time.

PhaseFocusKey tasksDone when
Days 1 to 30FoundationPositioning sentence, scope statement, 2 or 3 packages, prices, business structure, EIN, insurance quotes, coaching agreement, intake form, platform and Stripe connected, five-page websiteSomeone can find you, book a consult, sign and pay without you touching anything

How much does it cost to start a health coaching business?

A lean start can run on a software subscription, a domain and your time; a comfortable start adds an LLC, an insurance policy and professional help with your agreement and taxes. Fees for state filings, insurance and certifications vary too much to list, so get quotes for your situation.

Compare that with a stack built from separate tools: the four-tool combination in the earlier table already costs about $107 a month as listed in October 2026, and still has no habits, check-ins or client programs. Current Coachful plans are flat monthly, and lower plans cap the number of clients (Lite: $29/month for up to 5 clients; Solo: $49/month for up to 20 clients; Pro: $99/month with unlimited clients; Studio: $199/month with unlimited clients).

What mistakes do new health coaches make when starting a business?

Most early mistakes come from building before deciding, and from selling sessions instead of outcomes. These are the ones that cost the most time:

  1. Coaching "everyone who wants to be healthier". A broad niche makes every message generic. Pick a person and a problem, even if it feels narrow.
  2. Selling single sessions. Habit change takes weeks. Single sessions under-deliver for the client and keep you selling forever.
  3. Drifting outside scope. Writing medical diets, interpreting lab results or advising on medication puts clients and your business at risk. Refer out, and say so in writing.
  4. Underpricing to get started. A low price attracts less committed clients and makes the income math impossible. Use the pricing table above.
  5. Spending weeks on tools. Five tools, five logins and a week of integration before your first client. One platform, or a done-for-you setup, gets you live faster.
  6. Skipping the agreement. Without a signed agreement, refunds, no-shows and scope arguments become personal disputes.
  7. Waiting for a perfect website. Five pages and a booking button are enough. Talk to people while the site is imperfect.
  8. No plan for after the program. Offer ongoing support before the last session, not after the client has gone.

We cover more of these, with fixes, in 10 mistakes new health coaches make.

Frequently asked questions about starting a health coaching business

Do I need a certification to start a health coaching business?

Certification and licensing are different things. A certification is usually voluntary; whether any license applies depends on what you do and where, so check your state's rules, especially for nutrition advice. In practice, a recognized credential builds trust with clients and referral partners, and some employers and partners ask for one. Our certification comparison covers NBHWC, ACE and other programs.

Should I form an LLC for my health coaching business?

An LLC protects your personal assets from business liabilities in most instances, according to the SBA, while a sole proprietorship offers no separation. Many coaches start as sole proprietors to test the idea and form an LLC once income is steady. Because rules and fees vary by state, check with an accountant or attorney.

Can a health coach give meal plans?

It depends on where you work and what the plan is. General healthy-eating guidance and habit support are usually within a coach's scope, but some states restrict individualized nutrition advice to licensed professionals. Check your state's rules, and refer clients who need medical nutrition therapy to a registered dietitian.

How many clients do I need to make a full-time income?

It depends on your income goal and package prices. In our illustrative example, reaching $4,000 a month in gross revenue takes about 9 or 10 active clients on monthly or 12-week packages, before taxes and processing fees. Run the same math with your own target and prices.

Is Coachful HIPAA-compliant?

No. Coachful is not HIPAA-certified and does not sign a business associate agreement. It is built for independent coaching practices, not clinical record-keeping or insurance billing. If HIPAA applies to your work, use a tool that offers a BAA for that part of your practice.

What is Coachful's free white-glove service?

It is a done-for-you build included on every plan. You describe what you need (a website, a funnel, a program, an email sequence and more), attach your logo, photos and copy, and the Coachful team builds it inside your account, usually within about 24 hours. They never invent credentials, testimonials or prices, and they leave gaps for you to fill.

Can my clients use an app?

Yes. Clients get your branding inside the Coachful app, where they see their program, habits, check-ins, sessions and community. The app ships under the Coachful listing rather than your own App Store account, and it does not sync with Garmin, Fitbit, Whoop or Oura.

How long does it take to start a health coaching business?

Most coaches can complete the legal and tech setup in two to four weeks and sign their first clients within 30 to 60 days. The slowest part is usually building enough conversations to fill your first program, not the paperwork.

Do I need to pay estimated taxes as a health coach?

In the US, if you expect to owe $1,000 or more when you file, you generally need to make quarterly estimated payments, according to the IRS. Self-employment tax of 15.3% applies once net earnings reach $400. An accountant can help you set the right amount to put aside.

Can I run group health coaching programs?

Yes, and it is a good second step once you have a few one-to-one clients. In Coachful, enrolling clients in a program creates a cohort group automatically, and you can run group calls, webinars and a community feed alongside the program content.
